  • Initial session $192

    • A 75 minute session where we'll discuss your eating behaviors, health history, exercise routine, review your 24-hour food recall, discuss your goals and create a treatment plan.

  • Follow-up session $166

    • 45 minute sessions where we'll check in on goals set in previous sessions, find out what’s going well and what has been challenging.

Payment: Payment is due at the time of service. I accept credit cards and HSA/FSA cards. I offer a sliding scale/pro bono treatment as available. If this would apply to you, please visit Project Heal and request me as your practitioner.

Insurance: I am contracted with United Healthcare, Health Net and Medicare - for CKD and diabetes only. I can provide super bills for all other insurance companies. I am no longer in network with Anthem Blue Cross as of July 1, 2022.

 

“Right to Receive a Good Faith Estimate of Expected Charges” Under the No Surprises Act

Under Section 2799B-6 of the Public Health Service Act, health care providers and health care facilities are required to inform individuals who are not enrolled in an insurance plan or coverage or a Federal health care program, or not seeking to file a claim with their plan or coverage both orally and in writing of their ability, upon request or at the time of scheduling health care items and services, to receive a “Good Faith Estimate” of expected charges.
