  • Liz is in-network with United Healthcare and Health Net. She is happy to provide superbills for insurance plans she is not in-network with.

  • Prior to your initial session, Liz asks that you verify your insurance benefits. Unfortunately, not all plans cover nutrition services or they are limited to specific benefits and/or medical conditions. Please call your insurance and ask the questions below. Verifying your benefits ensures that you know how your policy works toward seeing a dietitian.

    • Make note of the date, time, representative’s name and a reference number for the call

    • What is my benefit period?

    • Does my policy cover nutrition counseling/medical nutrition therapy?

      • CPT codes 97802 and 97803

    • Is this benefit limited to a specific diagnosis (provide one if you have one)? Do I have preventative nutrition coverage?

    • Is a physician referral needed?

    • Do I have telehealth coverage?

    • Is there a limit to the number of visits?

    • Is there a limit to the number of units?

    • Does my deductible apply?

    • Do I have a copay/coinsurance?
